are you using a microsd card for storage?  sometimes I could be the fault of memory cards.  if you are using that, try taking it out and use internal and see if it makes a difference

I get this issue only if I use sd card for storage.

try a different memory card.  sometimes these memory cards can become corrupt over time.  good thing is memory cards are cheap to replace

Don't use them anymore. 64 GB phone no need for SD card. That was early in 2007-8 when phones doesn't have so much memory.
