Phones heats-up during charging.
Phones will further heat-up during fast charging especially 15W and above.
QC3.0 is a fast charging tech which does heat-up during operation. The behavior is rather expected based on its design than it being an issue.
The heating is a consequence of a fast charging process.
Further heating is due to usage while doing fast charging. Further load given (by using the phone) also increase heating, to maintain safe temperature limits which were dictated by the weakest part (battery), throttling is inevitable.
Safety issue? Phones reaching their temperature near their temp limit will lessen the charging power. If that does not lessen the heat (because the user is constantly using the phone during fast charging) the CPU will throttle. If both still does not alleviate the heat then the phone will shutdown. Safety measures for the phone were already been applied and satisfied so safety is NOT an issue here.
My advice would be for the user to not make it a habit to use the phone during fast charging. If usage is inevitable or is already a habit for the user, I advice to use a slow charger instead.

Well explained. the only point which bother me as this is not mentioned anywhere on the product itself and user should not ware of this. the specs of the phone says only this ...

Charger specifications and charging tests were done in a well controlled temperature setting. Mostly at 25°C ambient. Any deviations from that temp would have different results. Fast charging tests speeds and temperatures assumes that the phone is only being charge and not being used. If you add additional load to the charger (by using your phone while fast charging) and if you have higher ambient temp than 30°C, then I see no point on this being a problem. Fast charging will always produce more heat than slow chargers. If you really are concerned then can you provide:
Temperature value for the ambient temperature, the charger, and your phone during charging alone? This is for us to know if the temperatures were really are concerning.
